Resolved: Application Checklist email typo

The old Application Checklist (v6.1, Oct 2025) told applicants to email Case Log I & II to sfrier@ptcny.com, while every other current document — including that same checklist's own final page — uses eforms@ptcny.com. The corrected checklist linked above fixes both instances to eforms@ptcny.com.

Needs board resolution: Surgeon Attestation form is stale

The Surgeon Attestation form (undated, 2021 file) still lists PTC's old mailing address ("1350 Broadway, Suite 800") and instructs mailing a physical copy — every current document uses "25 West 39th Street, 8th Floor" and the email-based eforms@ptcny.com process instead. Unlike the checklist typo, this one needs a decision, not just a mechanical fix: should this form switch to the email-based process like everything else, or is physical mail still actually used for it?

Resolved: oral exam fee

Older versions of the Oral Application form (Version 5.5 Oct 2025, and a 2022 version still linked from PTC's own site) charged a separate $800 fee for a candidate's first oral sitting, which conflicted with the Written Application Instructions' claim that the first oral exam is already covered by the initial $1,600. Version 5.6 (May 2026) — now linked above — confirms there's no additional fee for the 1st oral exam (it's included in the $1,600); the $1,000 fee applies only to 2nd/3rd attempts. Still outstanding: PTC's own site still links to the stale 2022 version, and the live theabnm.org site (oral-exam page and Policy & Procedures Section II) hasn't been updated to match yet.
